I understand ADD Movement will be closed and requests will not be considered for the following dates: *Aug 31-Sept 4th *Sept 30th. Appointment opening notifications are not typically sent on weekends unless urgent short notice can be accommodated.

List the dates&/or time of day you are hoping to be notified about. The more detail, the better. All appointments will remain within the hours posted on the Home Page. Feel free to add any additional notes you would like to share re: the appointment needed

I understand that requests without specific dates will remain active for 3 weeks from the date submitted. (for example, “any day after 3:00 PM” or “any Monday”) After 3 weeks, a new request must be submitted to remain on the notification list.

I understand that the purpose of this form is to be notified of appointment openings. Submitting this form does not guarantee an appointment and will be contacted if something suitable becomes available. This form will be void if attempting to cancel a current appointment
